School Transition Plan

Transitions can be difficult for anyone at any age, from moving from elementary to middle school or moving to a new area, transitions can cause difficulties that can cause increase mental health issues. In the schools transitions happen quite a bit, from moving to another grade, to another school, to many different classes, to graduating. This time can be somewhat turbulent, however if careful planning and considerations are taken this experience can be positive and can lead to greater growth in an individual. As a professional school counselor it is our duty to work with parents, students and teachers to help students have as smooth of a transition as possible. The middle school transition can be an especially difficult one as students are facing puberty head on while also mediating social relationships. Students at this age are developing more independence and are given increased responsibilities. Students who are transitioning from middle school may have many difficulties with a more complicated schedule, opening lockers, remembering homework and more difficult class content. The school counselor alone can not fully prepare students however, they can create or utilize tools to ensure that teachers, parents and students are all ready to transition when the time comes. The middle school counselor can work with both the elementary and high school counselors to develop a set plan and create a timeline for when events and information should be disseminated. Above are two different transition plans, one for elementary to middle school and the other from middle school to high school. Both of these transitions can be difficult however, with preparation and support students can be successful through these huge life changes.
